(Form No. 07 – VT)
- Purpose: This table is used to reflect the total value of raw materials, supplies, tools, and equipment withdrawn from inventory during the month at actual and accounting prices, and to allocate the value of raw materials, supplies, tools, and equipment withdrawn for use to the users on a monthly basis (crediting accounts 152 and 153, and debiting related accounts). This table is also used to allocate the value of tools and equipment withdrawn for one-time use with a large value and a useful life of less than one year or more than one year, which are currently reflected in account 242.
- Recording methods and responsibilities
– The table consists of vertical columns reflecting the types of raw materials, supplies, and tools and equipment used during the month, calculated at accounting prices and actual prices; the horizontal rows reflect the users of the raw materials, supplies, tools, and equipment.
– Based on the material withdrawal documents and the difference between the accounting price and the actual price of each type of material, calculate the actual price of raw materials, supplies, and tools withdrawn from inventory.
The value of raw materials, supplies, tools, and equipment withdrawn from inventory during the month at actual cost, as reflected in the Material, Supplies, Tools, and Equipment Allocation Table for each user, is used as the basis for recording on the credit side of Accounts 152, 153, and 242 of the relevant statements, journals, and accounting books, depending on the accounting method applied by the unit (General Ledger or Journal-General Ledger Accounts 152, 153, etc.). The data from this allocation table is also used to aggregate costs for calculating the cost of products and services.

SUMMARY REPORT OF FIXED ASSET INVENTORY
(Form No. 05-TSCĐ)
- Purpose: The inventory record of fixed assets aims to confirm the quantity and value of existing fixed assets, identifying discrepancies compared to the accounting records. This serves to strengthen fixed asset management and establish a basis for assigning material responsibility and recording the difference in the accounting books.
- Recording methods and responsibilities
The top left corner of the Fixed Asset Inventory Summary Report clearly states the name of the unit (or unit seal) and the department using the asset. The inventory of fixed assets is conducted in accordance with legal regulations and the unit's requirements. An Inventory Committee must be established when conducting the inventory, with the fixed asset accountant as a member.
The inventory summary report of fixed assets must clearly state the time of the inventory: (… hour … day … month … year…).
When conducting an inventory, it must be done for each individual fixed asset item.
The "According to accounting records" entry, based on the fixed asset accounting records, must include all three indicators: Quantity, original cost, and remaining value in columns 1, 2, and 3.
The "According to inventory" entry is based on the results of the actual inventory count and must include all three indicators: Quantity, original cost, and remaining value in columns 4, 5, and 6.
The "Difference" section records the excess or deficit amount according to 3 criteria: Quantity, original cost, and remaining value in columns 7, 8, and 9.
The Inventory Summary Report of Fixed Assets must identify and clearly state the reasons for any surplus or shortage of fixed assets, and include comments and recommendations from the Inventory Committee. The Inventory Summary Report must be signed (with full names clearly stated) by the Head of the Inventory Committee, reviewed by the Chief Accountant, and approved by the enterprise director. Any discrepancies in the unit's fixed assets must be reported to the enterprise director for review.
6. Fixed Asset Depreciation Calculation and Allocation Table (Form No. 06 - Fixed Assets)
FIXED ASSET DEPRECIATION CALCULATION AND ALLOCATION TABLE
(Form No. 06 – Fixed Assets)
- Purpose: Used to reflect the amount of depreciation of fixed assets that must be recorded and to allocate that depreciation to the fixed asset users on a monthly basis.
- Structure and main content
The fixed asset depreciation calculation and allocation table has vertical columns reflecting the depreciation amount to be calculated for each fixed asset user (such as for the production department – accounts 623, 627, for the sales department – account 641, for the management department – account 642…) and horizontal rows reflecting the depreciation calculated in the previous month, the increase and decrease in depreciation, and the depreciation to be calculated in the current month.
Basis for establishment:
+ The depreciation expense calculated in the previous month is taken from the previous month's fixed asset depreciation calculation and allocation table.
+ The depreciation figures for fixed assets that increase or decrease this month are reflected in detail for each fixed asset, relating to the increase or decrease in depreciation according to the current regulations on fixed asset depreciation.
The depreciation amount to be calculated this month is equal to (=) the depreciation calculated in the previous month plus (+) the increase in depreciation, minus (-) the decrease in depreciation in the month.
The depreciation amount to be deducted this month on the Fixed Asset Depreciation Allocation Schedule is used to record in the relevant Statements, Journals - Vouchers and accounting books (credit column of Account 214), and is also used to calculate the actual cost of completed products and services.
